These modules on chronic condition management aim to meet the extended professional development needs of registered healthcare professionals involved in  delivery of services to people with or at risk of chronic diseases.Core principles and skills involved in developing and implementing self-management enhancing programs in diverse settings for health professionals and service users are central to the programs. The topics provide for learning across management of a diverse range of chronic conditions.

  • Possession of a UK or overseas degree from a recognized institution, or equivalent professional qualification.
  • Professional registration with bodies like NMC, HCPC, GMC, or relevant Allied Health Professional Body/Council.
  • Employment in a clinical role supporting the development of clinical skills during the programme.
  • Work experience in a clinical environment such as general practice, with preferably a qualified mentor.
  • Completion of prerequisite ELFH modules for some learning units.
  • Identification of a mentor before starting the course.
  • Understanding of common chronic diseases prevalent in primary care settings.
  • Recognition of risk factors, signs, and symptoms associated with chronic diseases.
  • Proficiency in conducting comprehensive assessments and formulating tailored management plans for patients with chronic conditions.
  • Application of evidence-based guidelines and treatment modalities for managing chronic diseases effectively.
  • Development of communication skills for patient education, counseling, and promoting self-management strategies.
  • Integration of multidisciplinary approaches in coordinating care and supporting patients with chronic illnesses.
  • Utilization of health technologies and resources for monitoring disease progression and optimizing patient outcomes.
  • Engagement in continuous learning and quality improvement initiatives to enhance care delivery for individuals with chronic diseases.

The assessment process for the Chronic Disease Courses is designed to evaluate participants' understanding and application of knowledge and skills related to managing chronic illnesses in primary care settings. Assessments may include written assignments, case studies, presentations, and practical assessments. Participants will be assessed on their ability to identify risk factors, recognize signs and symptoms, conduct comprehensive assessments, and formulate evidence-based management plans for patients with chronic diseases. Additionally, assessments will evaluate participants' communication skills in patient education and counseling, as well as their ability to collaborate within multidisciplinary teams to coordinate care effectively. Feedback provided during assessments will aim to support participants' learning and development, enabling them to apply best practices in managing chronic diseases and improving patient outcomes in their clinical practice.
